Lauria says the survivorship care plans created at the Cancer Institute of New Jersey are a blend of recommendations made by ASCO and NCCN and the electronic medical records that are kept at the institute. It is important to strike a balance by summarizing the patient’s treatment without duplicating the medical records.

For a survivorship care plan, Lauria says they focus on what drugs the patient received, what kind of radiation they received and the providers who administered the treatments, Lauria says. Creating a plan that contains a treatment summary and a follow-up care plan helps patients monitor their health, even if they move away.
