Nurses color code the electronic medical record to prevent errors. When the patient is first admitted, the nurse codes his or her chart with the color yellow. After the patient is discharged and the nurse finishes his/her chart and charges, the color status changes to blue, implying that it needs to be verified by a second nurse. Finally, if the second nurse agrees with the charges and chart and everything is entered correctly, the chart is closed and the color changes to red.
