The fall prevention project had two parts. The first was education, where pharmacists like Tsuker worked to help nurses better understand which drugs may lead to falls and how each drug works. The team then worked on a prevention plan.
The next step was pharmacy intervention and recommendation, where the team updated the screening tool to score each patient’s fall risk. Healthcare providers could then be sure to carefully watch those with higher risk, thus preventing falls.
